The report, which was obtained by People magazine, listed Simmons' cause of death as "sequelae of blunt traumatic injuries" that were caused by a "ground-level fall", with "arteriosclerotic cardiovascular disease" also cited as a "contributing condition".

"Per reports, he experienced an episode of dizziness and collapsed on the floor on the evening of July 11, 2024," the documents state. "He was found the next morning on July 12 and spent the day in bed. "On the morning of July 13, he was found unresponsive on the bedroom floor.

"His death was pronounced at the scene." Simmons' body was also found to have a fractured left femur as well as "incidental bilateral renal cortical cysts and a gallstone". The presence of antihistamine diphenhydramine, antidepressant trazodone, and zolpidem - which is used to treat insomnia - were detected but they did "not appear to have contributed to the cause of death".
