The family of a Stowmarket engineer said "sharing the horrific ordeal" the 27-year-old went through must be done in order to "highlight the litany of errors" that cost him his life. A report released into his death on Tuesday revealed system errors led to Jack Last receiving the AstraZeneca Covid-19 vaccine before he should been. In December 2022, a coroner concluded the 27-year-old died as a result of the Oxford AstraZeneca Covid-19 jab he was administered with.

A family statement released in the report said: "This statement isn't something any family would expect to make. However, sharing the horrific ordeal that Jack was put through is something that must be done to highlight the litany of errors that have cost Jack his life. "Jack was a happy, healthy, carefree 27-year-old.

He had thick brown hair, big brown smiling eyes and a kind smile. He loved seeing his family, his friends, keeping fit and exploring.

"He was witty, entertaining and always kind to everyone he met. Jack had previously taken a six month career break to work with machinery in Antarctica for the British Antarctic Survey, he completed several Tough Mudder challenges and could often be seen at his local park run. For Jack, life was worth living, and he made the most of every moment.
